Membership Agreement

 

Conduct & Behavior

I understand that my membership is contingent on adhering to the safety and ethics policies of PEAK Brazilian Jiu-jitsu. PEAK Brazilain Jiu-Jitsu can terminate this contract at any time and doing so will result in a prorated remainder of the monthly or paid in full tuition being returned. Being barred from PEAK Brazilian’ premises for any reason will result in a termination of membership, but the member will *not* receive a refund. It is the duty of members to follow and understand PEAK Brazilian Jiu-Jitsu & AmVet Post 13 Hall rules.

USE OF FACilities

Facility is defined to be spaces deemed to be in use by PEAK Brazilian Jiu-jitsu during scheduled hours of operations (please see ‘Classes’ page for more information on times) . These areas include bathrooms, waiting areas, parking lot, and mat space. PEAK Brazilian Jiu-jitsu & Amvet owners may from time to time change the rules governing the operations of the PEAK Brazilian Jiu-jitsu schedules of activities, and hours assigned. Members shall be provided with notice of these changes. Unauthorized use of facilities may result in membership suspension or cancellation. PEAK Brazilian Jiu-jitsu may suspend or cancel the rights, privileges, or memberships of any Member whose actions are detrimental to the enjoyment of these facilities by other Members, or for failure to comply with PEAK Brazilian Jiu-Jitsu & Amvet rules.

 

member’s rights to services

Membership is contingent of on time payment. A late fee of $10 will be applied to all memberships that have not paid tuition by the 6th of the month. If your account has been suspended, you will not be permitted to attend classes until it is current. Memberships can be put on hold due to injury, illness, hardship but must be discussed with PEAK | please reach out to Christian or Ed | info@peak-bjj.com

Furthermore, you agree that until such time as your account is current, that you forfeit the right to any services or amenities that would be normally offered or provided for by the terms of this agreement.

RIGHT TO CANCEL Agreement

If you wish to cancel this contract, you may do so with 30 days’ notice, or within 5 business days of the agreement’s signing via written or electronic communication to an authorized PEAK Brazilian Jiu-jitsu owner. By signing you understand that withdrawing automatic transactions can take up to 15 business days to process and revoking this agreement may still result in any pending ACH payments to continue to process. I agree that cancellation of my membership outside of the initial five-day grace period will not result in a refund.

  • KEEP COACHING OF OTHER STUDENTS DURING CLASS LIMITED.

  • NO FOUL LANGUAGE INSIDE THE ACADEMY.

  • IF YOU ARE LATE FOR CLASS, PLEASE EXCUSE YOURSELF AND WORK YOURSELF IN..

  • WHEN THE INSTRUCTOR IS DEMONSTRATING THE TECHNIQUES, STUDENTS MUST SIT OR STAND IN GOOD POSTURE.

 

WORK HARD.

 
  • TALKING SHOULD BE KEPT TO A MINIMUM AND SHOULD RELATE TO THE CLASS SUBJECT.

  • ALL STUDENTS & VISITORS MUST WEAR A CLEAN, WHITE/BLUE/BLACK/GREY/GREEN* UNIFORM.

    • Please discuss with Instructors if interested in purchasing a GI color not listed above.

  • ALWAYS WEAR YOUR SHOES OR SANDALS WHEN YOU ARE OFF THE MAT.

  • IF YOUR FEET ARE DIRTY, USE THE WIPES NEXT TO THE MAT TO CLEAN THEM BEFORE THE CLASS STARTS.

  • YOU MUST ALWAYS WEAR A SHIRT OR RASH GUARD INSIDE THE ACADEMY.

 

HAVE FUN.

 

  • ALL JEWELRY, PIERCINGS, NECKLACES AND OTHER ITEMS SHOULD BE REMOVED DURING TRAINING.

  • KEEP FINGERNAILS AND TOENAILS NEATLY TRIMMED.

  • SHOES, FOOD, DRINKS AND ELECTRONIC DEVICES ARE NOT ALLOWED ON THE MAT.

  • ALL TRAINING GEAR AND BAGS SHOULD BE LEFT OFF THE MATS.

  • BE COMMITTED, WORK HARD & HAVE FUN.
